This application is part of the ACC-AJRCA Accelerated Associate Membership Application Process. Upon your application being submitted, you will receive the exam study guide. While not all materials and fees are necessary to be turned in at the same, all materials must be turned in before your in-person exam. The application fee of $500 should be paid upon submission of this form.

By applying for Associate Membership in the ACC, I have read, and hereby agree to abide by the ACC's governing documents. I also understand that I am required to complete the two-year hadracha program to be elevated to Regular Membership in the ACC. I understand the rules regarding placement as governed by the JCPC.
